Verify that fields for Prevention to auto-populate into eForms are the following:
• Current date (same as the e-form date)

• Prevention Name (these will have link from the form checkbox to the same prevention name in our customized list)
• Provider Name (person doing to test)
• Initial status of ‘Pending’

Fields for Preventions are auto-populated
