Health Tracker Training Guide
Introduction to Health Tracker
Health Tracker allows providers to view and access all of the measurements and chronic disease management indicators available in a patient’s electronic health record, contributing to the management of their health. Health Tracker aims to replace the current implementation of the Measurements section, located in the eChart. By replacing the Measurements section with the Health Tracker, OSCAR will provide a centralized hub for providers to better manage measurements and indicators that are linked to chronic diseases, contributing to better health outcomes.
Accessing the Health Tracker
The Health Tracker is located under the Measurements section in the eChart.
To access > go to “Measurements” > selecting “Health Tracker” link.

Available Features
Warning: Measurement values cannot be deleted at this time. While the “Delete” button is there, this feature is not available. This bug will be fixed in the next upgrade.
Health Tracker includes a patient context heading that displays the following information:
Patient name (Last Name, First Name)
Link to MyOSCAR
Date of birth and age (Born: Year/Date/Month (age))
Sex
Health Information Number (HIN)

Health Tracker displays all flowsheets that are available in the system, and their respective measurements.
All flowsheets will appear in the top navigation bar. Flowsheets that are registered in the Disease Registry will appear with a green background and those that have not been registered with a diagnosis in the Disease Registry will appear with a grey background, depicted below.

When viewing a flowsheet that has not been registered in the Disease Registry, a warning will appear with the option to “Add Diagnosis” to the Disease Registry.

When viewing a flowsheet, a spark line graph icon will appear next to measurements that contain more than one value, providing an “at-a-glance” view of trending data for a given measurement or indicator. Clicking on this icon will open an expanded, detailed, graph.

An information deck, located on the right hand side of the Health Tracker, displays: Medications and Allergies recorded in the Prescription (Rx) module, Reminders from the eChart, and Warnings generated by the current flowsheet being viewed.
Please note: Measurements, Allergies, Reminders, and Warnings are not editable at this time. To add more information, data will need to be entered into the appropriate modules.

Improvements that will be made available in Health Tracker v2
Ontario MD requirements:
Adding visual indicator that the page is loading (i.e. when switching between flowsheets)
Ability to change targets/intervals for a single patient and overall for a provider/clinic
Ability to change which measurements/preventions appear on the flowsheet for a single patient and overall for a provider/clinic
Ability to print flowsheets
Ability to view entries from a specified date-range selection input by a user
Ability to view X amount of entries set by the provider (i.e. only show the last 3)
Ability to select which elements display (for printing)
