Pre-action requirements/state:

1. OSCAR and MyOSCAR (where applicable) are both configured and working as expected.

Post-action state:

1. System is behaving as expected after completion of the growth chart use cases/test cases.

Steps to perform the test case:

1. open OSCAR
2. search for a patient
3. click on their eChart
4. click on "Forms"
5. click on "add new form"
6. Growth Chart (0-36 months or > 2)

Failure Steps:

1. Unsuccessful creation addition of the growth chart.reation of third party bill for patient(s).  third party bill record is NOT saved for review or future edit.

Success Steps:

1. Expected result: successful creation of third party bill for patient(s).  third party bill record is saved for review or future editaddition of the growth chart.  Growth chart is saved in patients record for future use.
